Pathway Inclusion Worker
The post holder will be part of the Complex Psychosis Pathway and will work into the Level 1 and Level 2 Inpatient Rehabilitation units and in the community with service users that present with complex needs who are under the care of the Community Rehabilitation Team and Assertive Outreach Team. This is a Touchstone role based at both inpatient and community sites with multi-disciplinary and multi-agency teams with staff members from Touchstone, Leeds MIND, Inspire North, St Annes and NHS. To work across the Complex Psychosis Pathway caseload, in line with the multi-disciplinary teams, to identify appropriate community pathways for service users and reduce length of stay for service users in the rehabilitation and recovery pathway. To work with service users and carers to have a voice. Leading in co-production in the service, championing service user and carer involvement and engagement. To network with the vast community services across the city to develop a wealth of resources. To maintain knowledge of specialist agencies and pathways to welfare and benefit advice as required To hold and develop expertise in inclusive practice, reaching out to under-represented groups/communities, enabling opportunities for service users and stakeholders to access and enhance service provision. Workload will be continually negotiated through supervision and case load management with the appropriate clinical leads.
